International observational program report


  I’m general cardiology fellow from faculty of medicine, Prince of Songkhla University, Thailand. I am interest in cardiac electrophysiology such as advance catheter ablation. The Cardiac Electrophysiology at University of Tsukuba hospital is very famous. I have attended to the program for 3 weeks.

  In the beginning, I met Professor Ieda Masaki, Chief of cardiology department, Professor Akihiko Nogami, World’s Famous electrophysiologist. They gave me very warming welcomed, and they are very nice to me. I’d like to thank for the opportunity of accepting me in the program.

  During the training program, mainly I was in the Electrophysiology (EP) laboratory every day. The center has the EP teams to do the EP study and treatment for the cardiac arrhythmias including cardiac electrophysiologist(doctors), nurses and technicians. In the lab, they use some equipment which useful and only available in Japan. The patients’ diagnosis was complex, difficult, and rare so the procedure for treatment also was complicated and need skillful doctors.

  I observed in many procedures such as pulmonary vein isolation (PVI), VT ablation and EPS for SVT, etc. High density mapping is mainly used in every procedure for the accuracy and safety of the patients. Professor Igarashi always give a good recommendation and technique of PVI. In every case, I have learned the new knowledge, technique of the procedure and some discussion about the case with the staffs, other fellows, and residents is profitable.

  The most impressive case is the case of near-zero of fluoroscopy during PVI. This is the first time I saw the procedure. The EP team inserted and placed the catheters into the cardiac chamber by using the intracardiac echocardiogram (ICE). The procedure can reduce the fluoroscopy time which is better.

  This is my first-time study abroad, and it was the great experience, memorably and vey impressive. Unseen and rare cases of catheter ablation were very fascinated and all gives me a lot of inspiration.
