For first-time patients

(1) Place :Desk #1 at the Keyaki building
(2) Time :30 minutes before your appointment time
(3) What to present :
     National Health Insurance Card,
     your reference letter, any other important medical records (CDs),
     ※Photo identification (Residence Card or Special Permanent Resident Certificate or Passport),
     some kind of verification document for your katakana name.
(4) Flow:
     ① Take a Waiting Number Ticket
     ②Fill in the New Patient Registration Form (Green paper on the desk).
     ③When your waiting number is called, present the New Patient Registration Form with above (3).

The “New Patient Registration Form ” can be downloaded from here.
Please print it out, fill in the necessary information, and bring it with you on the day of your visit.

Notes on the first visit

  • Do you have a reservation?
    If you do not have an appointment, we may not be able to see you on the same day.

  • Did you receive your prescriptions, medical certificates, and other documents at the reception desk after your visit?
    If not, please come to checkout desk 10.
  • Do you need a prescription for medication?
    If you need your prescription filled in the hospital, please come to the Pharmacy Department.

Flow of the first visit

For first-time patients

If you are visiting the University of Tsukuba Hospital for the first time, please fill out the Application Form for Initial Medical Examination, and submit your insurance card (various types of beneficiary certificates) and a letter of introduction (CDs are also accepted) to the receptionist at No. 1.
